Remote monitoring results submitted by the patient will be reviewed by the surgery to measure your results and maintain that we provide the correct assistance for your long-term condition.

You will then be able to use our online remote monitoring form to submit your reading securely online to the Surgery. All readings will be seen by your Doctor and action will be taken if need be.
